Start with the date of writing the letter. Look at some sample dates:

What if the date is not directly provided in the case notes?

If the date of writing is not provided in the case notes:

• Write the date of exam.
• Write the date of the patient’s last visit.

Where to find the Date of Writing?

• At the starting of the case notes, usually in the first 10 lines.
• In the Discharge Plan areas of the case notes.
• The last visit date.
• Last of the “progress date.”
